Some notes on getting the IPW2200 working nicely with rfmon and current kernels:

1.  Get the latest IEEE stack (1.1.9) from http://ieee80211.sourceforge.net

2.  Get the latest IPW2200 drivers (1.0.10) from http://ipw2200.sourceforge.net

3.  Download the IPW2200 2.3 firmware (which is NOT the latest) from the ipw2200 site

3.  Install stack

4.  Edit the IPW2200 Makefile and uncomment "CONFIG_IEEE80211_RADIOTAP=y"

5.  Edit ipw2200.c and change "#define IPW_FW_MINOR_VERSION 4" to "#define IPW_FW_MINOR_VERSION 3" around line 2930

6.  Compile and install drivers and firmware

There are known to be problems with monitor mode and the 2.4 firmware which result in missing frames, high system load, and floods of restart errors in dmesg.  These instructions will enable the radiotap per-packet signal headers and tell the drivers to use the older 2.3 firmware set.  There shouldn't be any impact in normal operation by using these.
