Dropping dead in rfmon w/ no notification is very very common behavior with that driver. I know of no fix other than to try to backlevel to an earlier orinoco driver from outside the kernel tree, or MAYBE mess with firmware versions (which is usually non-trivial and requires a windows machine for flashing)
